A Message from the AFTA Membership Chair, Carmen Knudson-Martin, Ph.D.
Below you will find the application and recommendation forms for membership in AFTA. Please carefully review the Membership Criteria and select your references accordingly. It will be your responsibility to contact the people you wish to serve as your references and provide them with the enclosed recommendation forms. People providing references should return them directly to AFTA's Central Office.
Occasionally, in the face of extraordinary contributions, AFTA waives some of the usual requirements. If you feel you should be considered for such a review, please make a note to that effect on your application form.
To apply for Regular or Ear Career Membership in AFTA, please send us the following:
- Application Form for Regular Membership or Early Career Membership.
- Letters of Recommendation (see criteria for number needed, Regular Membership Recommendation or Early Career Membership Recommendation)
- A recent C.V. and additional pages, if necessary
- $20.00 Application Fee Payable to AFTA
To apply for Student Membership, please send us the following:
- Application Form
(Student Membership)
- Letters of Nomination (Must be from an AFTA Member, Student Nomination)

- If you downloaded the Word document, please fill in your answers and send in the document by return e-mail in an attachment. If you downloaded the PDF document, please handwrite your answers in clear print and return by regular mail or fax.
